E4C is seeking a Follow-up Support Worker in Edmonton, AB.
Are you a relationship builder, team player and are passionate about supporting individuals in establishing
stability in their housing communities? The e4c Rapid Re-Housing and Housing First Team is part of the
service delivery team with Homeward Trust Housing First in Edmonton that empowers participants in
maintaining tenancy. You will provide resources to aid in housing stability through a collaborative
exploration of a participant’s strengths, barriers and goals.
You will work with persons served of a variety of demographics. You may work within various program
models concurrently, including but not limited to 3 month Rapid Re-Housing model and 1 year Housing
First model. You offer on-going support to participants through regular case management meetings, case
plan development and support, and the creation of housing goals with participants. With your knowledge
of harm reduction and trauma informed practices, you will manage a caseload up to 20 participants in
achieving housing stability and providing them with tools to access community based funding and
resources.
